Advanced Cryoablation for Renal Tumor Successfully Performed at Sir Ganga Ram City Hospital

### *Clinical Case Summary*

*Patient:*
A 62-year-old male, known case of bilateral renal lesions of neoplastic etiology, presented with a heterogeneously enhancing lobulated mass measuring 9.6 x 7.7 x 6.5 cm in the left upper pole and interpolar region with extension into the renal hilum, abutting the pelvicalyceal system. A secondary lesion measuring 18 x 17 x 15 mm was identified in the right interpolar region. The patient had previously undergone a left radical nephrectomy and was planned for definitive minimally invasive management of the right renal lesion.

### *Procedure Details*

Following thorough pre-procedural optimization and informed consent, *USG and CT-guided percutaneous cryoablation* of the right renal lesion was performed under general anesthesia with strict aseptic precautions. Two 17G x 17.5 cm cryoablation probes were deployed, utilizing a *Boston Cryo System* for precise thermal ablation. The freeze–thaw–freeze protocol was applied (10 min freeze – 8 min thaw – 8 min freeze) to ensure complete cytotoxic necrosis of the tumor tissue while sparing surrounding parenchyma.

Intra-procedural imaging confirmed appropriate ice-ball formation encompassing the entire lesion with adequate ablative margins. Final tract ablation and dressing were performed post-procedure. The intervention was uneventful, with no peri-procedural complications.

### *Outcome*

The patient tolerated the procedure well and was monitored post-operatively in the recovery unit. He was discharged on the third post-procedural day in a stable and satisfactory clinical condition.

This case exemplifies the safe and effective use of *image-guided percutaneous cryoablation* as a nephron-sparing, minimally invasive treatment for select renal neoplasms, especially in patients unfit for repeat major surgery.
