Department of Neurosurgery is a superspecialty department with 12 consultants. It has two general neurosurgery units, a vascular surgery and interventional neuroradiology unit, spine surgery unit, and a paediatric neurosurgery unit. The department is well equipped to treat all type of emergency and elective neurosurgical problems including head and spine injuries, brain tumours, strokes, aneurysm, AV malformations, congenital disorders like hydrocephalus, spinal dysraphisms, spinal tumours, disc disorders, treatment of trigeminal neuralgias, Parkinson's disease, aneurysm coiling, carotid stenting, angioplasty, etc.
The department has state-of-the-art neuroendoscopic system, stereotactic system, operating microscopes, ultrasonic aspirator, new seamless operation theatres, ICU and post operative ward.
Head injuries and other emergency services: The consultants and residents are available round the clock to attend all type of head & spine injuries, brain haemorrhage, strokes and other neurosurgical emergencies. Operation facilities are available round the clock. Serious head injury patients are taken into ICU for close monitoring and ventilatory support if required. Traumatic haematomas like significant extradural haematoma, subdural haematomas, brain contusions are immediately removed surgically. Large hypertensive intracerebral haematomas are aspirated stereotactically.
Brain Tumour surgery: All type of brain tumours are removed microscopically. Examples of brain tumours operated are meningiomas, pituitary tumours, craniopharyngioma, glioma, hemangioblastoma, ependymoma, acoustic tumours, Trigeminal neuromas, base of skull tumours, skull tumours, orbital tumours, intraventricular tumours etc.
Vascular surgery: Clipping of all type of aneurysms of both anterior and posterior circulation, small to giant aneurysms, surgery of arterio-venous malformations, carotid endarterectomy, arterial bypass surgery, etc., is routinely performed.
Spinal surgery: Surgery of all type of spinal tumours viz. bony, extra dural tumours, intra dural tumours, intarmedullary tumour, etc. Spinal fixation of various types viz. anterior, posterior at all levels as per requirement is done. Microsurgery for cervical and lumbar disc surgery is performed. Endoscopic lumbar disc surgery is performed on a day care basis.
Paediatric neurosurgery: All type of congenital, neoplastic, traumatic, vascular problems are dealt with. Hydrocephalus, craniostenosis, spinal dysraphism, Chiari malformations are operated. All type of paediatric brain tumours like craniopharyngioma, gliomas, ependymoma, medulloblastoma, etc are operated microscopically.
Endoscopic Brain surgery: Endoscopic transnasal surgery is performed for Pituitary tumours as a minimally invasive procedure. No postoperative nasal packing is performed and most of the times, patients are discharged next day following surgery. Endoscopic third ventriculostomy is performed for treatment of hydrocephalus and this gives a permanent cure to many such patients. Endoscopic transnasal surgery is performed for CSF rhinorrhoea. Patients don't loose sense of smell after surgery, hospital stay is shortened. Many other intraventricular tumours like colloid cyst, cysticercus, etc are operated endoscopically.
Endoscopic Disc surgery: Lumbar disc surgery is a very common problem. Surgery is required where medical treatment fails to relieve pain (sciatica) or if patient develops weakness in leg or there is loss of control over urine and stool. Nowadays surgery for lumbar disc can be performed endoscopically through a tiny (around 1.5 cms) incision in the back. Patients are allowed to walk the same day and also sent back home same day. This surgery is also commonly called as Band-Aid surgery.
Stereotactic and functional neurosurgery: Stereotaxy is the method of localisation of lesion/target in the brain in three dimensions with the help of a special frame and surgery is then performed through a small hole in the skull. Stereotactic surgery is performed for taking biopsy from deep-seated lesions in the brain, for aspiration of brain abscess, intracranial cysts like craniopharyngioma cysts etc. Hypertensive intracerebral haematomas are aspirated stereotactically through a small hole in the skull under local anaesthesia. Functional neurosurgery, deep brain stimulation, pallidotomy and thalamotomy are performed as per indications. Epilepsy surgery including vagal nerve stimulation is also performed.
Peripheral Nerve surgery: Surgery of brachial plexus and other peripheral nerves is performed under microscope. All type of traumatic, neoplastic, entrapment disorders of nerves are dealt with. Carpal tunnel syndrome is operated under local anesthesia on a day care basis.
Interventional Neuroradiology: Coiling of aneurysms, embolisation of intracranial and spinal AVMs, AV fistulas, embolisation of tumours, stenting of intra or extracranial vessels e.g. carotid angioplasty, cerebral and spinal angiography are available.
Pain & Spasticity surgery: Radiofrequency ablation for trigeminal neuralgia, facet joint ablation, microvascular decompression for trigeminal neuralgia, glossopharyngeal neuralgia, intra-thecal baclofen pump implantation for spasticity is performed.
Teaching Programme: The department has a postgraduate teaching and research programme. There is a five-year DNB course and one student is selected every year, 4 students have already cleared the course and are established in the field of neurosurgery in different cities of the country. There are 5 postgraduate students and 2 senior residents in the department. Regular academic meetings are held of Neuroscience department where case discussions, seminars, group discussions, journal clubs, lectures and demonstrations are held.
